Who this is for
Adults whose plan reviewer or Support Coordinator has requested updated functional documentation. Most often for participants with neurological conditions, psychosocial disability, or progressive physical conditions where capacity has changed since the last plan.
What's included
- Two-session functional assessment in the participant's typical environment.
- Standardised measures appropriate to the referral question.
- Interview with the participant and (with consent) a nominated support.
- Written FCA report aligned to the NDIA's evidence requirements.
How it usually runs
1. Referral and scope
Coordinator or participant emails through the request. I confirm scope, timeline, and price within 48 hours.
2. Assessment sessions
Typically two sessions in the participant's home, with optional community observation if relevant to goals.
3. Report
Full FCA report delivered within 2–3 weeks. Includes function-by-domain summary and explicit support recommendations.
4. Plan-review readiness
Short follow-up call with the Coordinator to confirm the report addresses the questions the plan reviewer is likely to ask.
Reports
Full NDIS-aligned FCA report. Length typically 20–35 pages depending on complexity.

Charged per the current NDIS Price Guide line items for OT assessment and report writing.
