Who this is for
Participants where small environmental changes meaningfully reduce risk or open up participation. I do not scope complex home modifications - those require a specialist team and I will refer.
What's included
- Home visit and risk-based scoping.
- Written recommendations with line-by-line clinical justification.
- Coordination with builders and suppliers for quotes.
- Post-installation review.
How it usually runs
1. Home visit
Functional assessment in the spaces of concern, with the participant present.
2. Scoping report
Written recommendations and a short justification per item.
3. Quotes
I'll coordinate with one or two trusted local trades for written quotes.
4. Post-install review
A short follow-up to confirm the modification is working as intended.
Reports
Minor home modification scoping report with itemised justification.
