How I scope these
I start with a 30-minute call with the Coordinator or participant. The aim is to understand what's actually changed since the last plan and what the upcoming review will need to address.
A well-scoped FCA addresses specific questions; an unscoped one becomes a generic 200-item checklist that doesn't help anyone.
Process and timeline
Two assessment sessions, typically in the participant's home. Optional community observation where it's relevant to the goals.
Report delivered within 2–3 weeks of the final session. I'll do a short Coordinator debrief if helpful before the plan-review meeting.
