Why this matters
Paediatric goals - school participation, fine motor, family routines - are not the goals an adult plan funds. Plans rebuilt at 16 or 17 without that shift in framing often lose support hours.
What to start asking earlier
What does adult participation look like for this young person? Employment, education, independent living, community? What capacity is on the trajectory to build, and what isn't?
An FCA framed around adult goals - not extended paediatric ones - sets the next plan up much better than one that hasn't made that shift.
